Instructions for filling
out forms with Adobe Reader 8: |

1. |
It is recommended that you save
the forms below to your computer's hard drive prior to filling them
in. To download the forms to your computer's hard drive, place
your cursor over each form, right click your mouse and choose the
"Save Target As" option and select a destination on your computer's
hard drive to store the form files. |

From the toolbar located at the
top left corner of the Adobe Reader program, select "File" and "Open"
,
then locate and open the form that you want to fill out. |

If the hand
tool
is not displayed on the top toolbar, select "Tools" and then select
"Customize Toolbars" located at the bottom of the menu options. |

4. |
Scroll down to the "Select & Zoom
Toolbar" options and check the "Hand Tool" and then click "OK" to
close the "Customize Toolbars" dialog box. |

5. |
Select the hand
tool
on the toolbar at the top of the
Adobe Reader program. |
|
6. |
Position the pointer inside the form
field, and click. The I-beam pointer allows you to enter text. |
|
7. |
After entering text or selecting an item,
do one of the following: |

Press Tab to accept the form field change
and go to the next form field. |
|
|
Press Shift+Tab to accept the form field
change and go to the previous form field. |
|
|
Press escape to reject the form field
change and deselect the current form field. |
|
8. |
Once you have filled in the appropriate
form fields, proof the form for accuracy, save the form and then print
the form. |

IMPORTANT: |
You will not be able to save
the form field data unless you have the full Abode Acrobat
Professional version 5 or higher installed on your computer with the
ability to export form field data or unless you are using Adobe
Reader version 8.
If you are NOT using one of
the products mentioned above, your printed
documents will become your originals and once the file is
closed, the information that you typed in will be lost. |
