may also change your driver license/identification (ID) card address record in person at your local
Deputy Registrar's
office (please present
proof of new
Ohio residence street address), or in writing to the BMV. Written requests should include your full name, date of
birth, Ohio driver's license or ID card number, previous address, and your new address.
Upon subsequent issuance/renewal of your driver license or ID card at your local
deputy registrar, you will be required to submit
proof of the new
Ohio residence street address. The address change request should
