How do I obtain a Disabled Veteran License plate?

You must complete form BMV 4507 and submit it to our office along with a current letter from the VA that states you are 100% disabled or unemployable due to a service connected disability and either a copy of your current registration card or the original certificate of title or memorandum title.

Can I have more than one Disabled Veteran License plate?

Only one disabled veteran license plate can be issued to an individual unless you have received a monetary allowance from the VA to modify your vehicle.

How much do disabled veteran license plates cost?

There is no fee for a disabled veteran license plate.

I purchased a new vehicle, how do I transfer my Disabled Veteran license plate?

You must complete form BMV 4506 and submit it to our office along with the original title or memorandum title to the new vehicle.

My spouse who was the disabled veteran is deceased; can I keep the license plates?

Disabled veteran license plates can only be issued to the veteran. You will need to go to your local license agency and purchase a regular set of license plates.

Does my vehicle still need to be E-Checked?

Yes. You will be contacted by the Ohio EPA regarding testing. For additional information you can call the EPA at 1-800-CAR-TEST.

As a Disabled Veteran, what else do I qualify for?

As a disabled veteran you also qualify for a free driver's license. Take a current letter from the VA stating you are 100% disabled due to a service connected disability to any local license agency.

You may also qualify for a free hunting and fishing license. Contact the Ohio Department of Natural Resources at 1-800-WILDLIFE (1-800-945-3543).

How do I obtain a Former Prisoner of War license plate?

You must complete form BMV 4507 and submit it to our office along with one of the following: (a) documentary evidence stating your Prisoner of War status from one of the Armed Forces of the U.S., (b) telegram advising family of PW status, (c) Letter from the War Claims of Commission, (d) Letter from U.S. Department of Veterans Affairs and either a copy of your current registration or the original certificate of title or memorandum title.

I purchased a new vehicle, how do I transfer my former POW license plate?

You must complete form BMV 4506 and submit it to our office along with the original title or memorandum title to the new vehicle.

How much do Former Prisoner of War license plates cost?

There is no fee for a Former Prisoner of War license plate.

My spouse who was the former prisoner of war is deceased; can I keep the license plates?

Yes, you will need to submit a copy of the death certificate to do the transfer.

How do I obtain a Congressional Medal of Honor License Plate?

You must complete form BMV 4507 and submit it to our office along with documentary evidence in support of the award and either a copy of your current registration or the original certificate of title or memorandum title.

How much do Congressional Medal of Honor license plates cost?

There is no fee for a Congressional Medal of Honor license plate.

How do I apply for a Public Owned license plate?

Must complete form BMV 4501 and submit it along with the original certificate of title or memorandum title.

How do I transfer or replace a public owned license plate?

Complete for BMV 4501 and submit it to our office. If applying for a transfer, you must also submit the original certificate of title or memorandum title.

Do public owned vehicles need to be E-checked?

Yes, vehicles registered in an EPA county require testing. You must report to the EPA by December 31 in odd numbered years.
