Proof of Insurance Documentation

1. A policy of insurance in effect that meets the minimum standards under Section 4509.51 of the Ohio Revised Code as listed below:

• $12,500 because of bodily injury to or death of 1 person in any one accident.

• $25,000 because of bodily injury to or death of 2 or more persons in one accident.

• $7,500 because of injury to property of others in any one accident.

• Proof, to the satisfaction of the Registrar, that person is able to respond to damages in an amount at least equal to the above figures.

2. A copy of the declaration page of a policy of liability insurance.

3. Form BMV 2262, 2263 or 2294 completed by insurance agent (Back of Suspension Notice).

4. A liability bond.

5. A financial responsibility identification card issued by insurance company.

6. A binder of liability insurance bearing the original signature of an authorized agent and valid for no more than 30 days from date of issuance.
