How Do I Self-Certify?

Drivers can self-certify:

PUCO Provisional Waiver

Commercial drivers must meet minimum medical standards established by federal and state rules and regulations. The Public Utilities Commission of Ohio offers Provisional Medical Certification for eligible intrastate Ohio drivers whom are unable to pass the physical examination required by USDOT

The medical waiver and CDL license must be carried at all times when operating a commercial motor vehicle.

More information about Provisional Medical Certification can be found HERE.
